Responsibilities
With guidance from the Manager of Population Health Management, the
RN Population Health Coach works in collaboration and continuous
partnership with patients, their family/caregiver(s), providers,
clinics, specialty providers, staff and other healthcare
professionals to provide individualized care management across the
continuum of care for patients within the clinically integrated
network (CIN), Arkansas Health Network. The RN Population Health
Coach is responsible for coordinating a wide range of
self-management support and disease registry activities in
collaboration with healthcare team members to ensure safe, quality
and cost effective care.
Qualifications
Associate Degree in Nursing with RN license plus 5 years relevant
experience required; Bachelor of Science in Nursing or Master of
Science in Nursing preferred
Five years of clinical, community, public health, case management,
or health coaching experience required.
Nursing education experience preferred.
Experience working with Medicare and Medicaid populations and
related social/economic issues preferred; post-acute or wound care
experience preferred.
