Job Description Job Description PRN Days LPN - Hospice Overview: This role focuses on delivering skilled nursing care to hospice patients in their residences, operating under the supervision of a Registered Nurse. Responsibilities include assessing, planning, implementing, and evaluating the care needs of patients. Applicants should have completed an accredited practical nursing program, possess a valid LPN license, and have a minimum of one year of nursing experience. The position demands physical endurance, a valid driver's license, and may necessitate additional certifications such as IV certification and CPR. Discover your purpose with us! In this role, you will be responsible for providing skilled nursing services to patients in their homes, guided by a Registered Nurse. You will assess, plan, implement, and evaluate the care and needs of patients. Position Details: Minimum Qualifications: Licensure: Must be a graduate of an accredited practical nursing program; equivalent educational experience and successful licensure may also be considered. It is essential to obtain and maintain a current LPN license in the state where nursing duties are performed, adhering to all State Board of Nursing regulations. A valid driver's license is required. Experience: At least 1 year of nursing experience. Certification(s): CPR (Cardiopulmonary Resuscitation) certification is mandatory. Preferred Qualifications: Certification(s): Note: Depending on the specific position or unit, one or more of the following certifications may be required, or certification must be obtained within a specified timeframe: • IV Certification within one year of hire • Additional unit-specific certifications as necessary. Physical Requirements: • The role necessitates the ability to regularly push, pull, and/or lift up to 50 lbs. • Prolonged standing and walking are required throughout each shift. • The position involves gripping, reaching, bending, kneeling, twisting, and squatting to fulfill job duties.
