Job Description Job Description About the Role: The Licensed
Practical Nurse (LPN) at New Life Center will play a critical role
in delivering high-quality nursing care during the overnight shift
from 7 PM to 7 AM. This position is essential in ensuring the
health, safety, and comfort of patients by providing direct nursing
care, monitoring patient conditions, and collaborating with
healthcare team members. The LPN will be responsible for
administering medications, performing routine assessments, and
responding promptly to patient needs and emergencies. This role
requires a compassionate and detail-oriented professional who can
work independently while maintaining clear communication with the
multidisciplinary team. Ultimately, the LPN will contribute to the
overall well-being and recovery of patients by adhering to
established protocols and promoting a supportive care environment
throughout the night shift. Minimum Qualifications: Current and
valid Licensed Practical Nurse (LPN) license in the state of
employment within the United States. Completion of an accredited
practical nursing program. Basic Life Support (BLS) certification.
Demonstrated ability to provide compassionate and competent nursing
care. Strong communication and documentation skills. Preferred
Qualifications: Experience working in a long-term care,
rehabilitation, or similar healthcare setting. Additional
certifications such as IV therapy or medication administration
endorsements. Familiarity with electronic health record (EHR)
systems. Ability to work independently during overnight shifts with
minimal supervision. Experience in patient education and family
support. Responsibilities: Provide direct patient care including
administering medications, wound care, and other treatments as
prescribed. Monitor and document patient vital signs, symptoms, and
changes in condition during the night shift. Assist patients with
activities of daily living such as hygiene, mobility, and feeding
as needed. Collaborate with registered nurses, physicians, and
other healthcare professionals to develop and implement
individualized care plans. Respond promptly to patient emergencies
and communicate effectively with the healthcare team to ensure
timely interventions. Maintain accurate and thorough patient
records in compliance with regulatory standards and facility
policies. Educate patients and their families on health maintenance
and post-discharge care instructions. Ensure a safe, clean, and
comfortable environment for patients throughout the overnight
hours. Skills: The required skills enable the LPN to deliver safe
and effective nursing care by accurately assessing patient needs
and administering treatments according to established protocols.
Strong communication skills are essential for coordinating with
healthcare team members and documenting patient information clearly
and thoroughly. Preferred skills such as proficiency with
electronic health records enhance efficiency and accuracy in
maintaining patient data. The ability to work independently during
overnight hours requires strong critical thinking and
decision-making skills to respond to patient emergencies promptly.
Additionally, interpersonal skills support compassionate patient
interactions and effective education of patients and their
families, contributing to improved health outcomes.
