Position Type: Regular Scheduled Hours per 2 week Pay Period: 80 Primary Location: AR > LITTLE ROCK > SV DIABETES AND ENDOCRINOLOGY JOB SUMMARY: In accordance with our mission and core values of reverence, integrity, compassion and excellence, this position is established to serve as a consultant/resource professional in specialized areas of nursing. Teamwork is a vital component of patient care; therefore, incumbents will be expected to demonstrate their commitment to same through effective collaborative efforts. ESSENTIAL FUNCTIONS: 1. ASSESSMENT AND EVALUATION: Assesses educational needs through a variety of methods both formal and informal for patient, staff and/or community education. Conducts evaluations of all projects and education for desired outcomes and revises future work goals, methods and care given. Annually conducts educational needs assessment using a variety of methods which reflect insight into situation. 2. PLANNING AND/OR ORGANIZATION: Develops curriculum and action plans to meet identified needs of patient staff and/or community. Defines and incorporates all important information in action plans to meet identified needs. Assures plan is designed to meet needs of target audience. Draws on professional journals, literature, research and peers for innovative and successful programs. Utilizes evidenced based research and practice standards when developing programs. Communicates action plans promptly and professionally to target audience. Works collaboratively with peers, Department Managers, and Risk Management to identify needs and plan programs that enhance the competencies of staff in area of responsibility. Develops, facilitates and evaluates orientation program for area of responsibility annually; incorporating areas identified by JCAHO, Risk Management and Department Managers. 3. INTERVENTION AND PROJECT MANAGEMENT Utilizes effective intervention techniques and project management tools to successfully access, plan, contract and merge multiple projects simultaneously. Implements action plans which maximize resources to achieve outcomes. Utilizes all resources available to effectively manage multiple projects simultaneously. Demonstrates and maintains clinical expertise in area of specialty. Participates in development of standards of practice, policies, procedures and programs to enhance quality of patient care, including performance improvement plans. Supports and promotes well-designed research in specialty area and bases clinical practice on such research. 4. GROUP FACILITATION AND TEAMWORK Facilitates goal achievement and group dynamics in inter/intradepartmental and system wide projects. Models productive team member behavior in all activities. Uses key learning and group process principles Provides and elicits feedback keeping feedback concise and focused. Effectively manages groups of individuals (co-workers, patients, activity participants). Models productive team member behavior in both interdepartmental and intradepartmental activities. Takes an active role in leading and participating in committees and/or groups Facilitates development of leadership, critical thinking and decision-making skills. Facilitates the development of new team member skills for successful balancing of needed skills among team. 5. DATA MANAGEMENT Creates manageable and useful documentation of all projects, education and programs. Creates and maintains manageable and useful documentation of all projects, education and programs. Maintains current records of staff competencies in area of responsibility. Uses data collection process as teaching tool for staff to build assessment and evaluation skills Makes use of any appropriate existing data being collected for evaluation purposes. 6. CLINICAL PRACTICE Collaborates with other health professionals to develop/facilitate efficient and cost effective care for individual patients or groups of patients with related diagnoses. Collaborates with other team members to ensure that appropriate patient/family education is provided and documented. Serves as resource person to health team members. Demonstrates clinical expertise in area of Diabetes managementProvides classes in both Little Rock and North Little Rock during routine hours as well as some after hour classes to meet patients needs that include areas of A1C, weight tracking, quality of life, etc. Assists in planning, implementing and evaluating the Diabetes Self Management Education (DSME) program. Assists with the day to day management of the program as well annual curriculum review and adhering to all requirements of the ADA to maintain recognition of the program. Assists in marketing program and being actively involved with the advisory committee. Maintains certification as a Certified Pump Trainer for Animas, Medtronic and Omnipod. Educate, train and follow patients on pump therapy. In consultation with the physician, assists with adjusting settings and basal rate testing. Trains patients on continuous glucose monitoring systems (CGMS) both diagnostic and continuo MINIMUM QUALIFICATIONS: Education: Graduate from an accredited School of Nursing and current RN Licensure in Arkansas. Attain and maintain national certification as a Certified Diabetes Educator (CDE). Experience: One year of Diabetes Education experience. Physical Requirements: This job requires, at various times, the following physical activities; climbing, balancing, stooping, kneeling, crouching, reaching, standing, carrying, walking, pushing, pulling, lifting, finger dexterity, grasping, feeling, talking, and hearing, sometimes exerting in excess of 20-100 pounds of force. The worker is required to have close visual acuity to perform daily activities. May be subject to inside environmental conditions, hazards (including electrical currents and exposure to high heat and chemicals), infectious diseases, and may be required to function around prisoners and mental patients. Skills: Must demonstrate competence in all required area-specific skills. Possess effective communication skills, able to deal effectively with multi-disciplinary team members, patients, significant others and physicians. Must demonstrate effectiveness in computer processes and knowledge of data processing and applicable computer skills. Provides Diabetes classes to all levels of patients from children to adults.
