Job Description Social Worker A well-established hospital located near Marshall, Minnesota, is seeking a Social Worker to join its exceptional team. Offering $28.00 - $32.00/hour $5,000 Relocation Assistance Open to New Grads Social Worker Opportunity: Full-Time, Permanent, Direct-Hire Schedule: 8 am - 4:30 pm Reports to Hospital DON and Hospice Manager Includes a Comprehensive and Robust Benefits Package that includes Medical, Dental, and Vision Social Worker Responsibilities: Be a core member of the Hospice team and provide support to patients and families' emotional, social, and economic needs relating to the impact of a terminal illness Plan the transition and transportation of patients from hospitals to a home or another facility, arrange services, and collaborate with doctors, nurses, and insurers Plan and lead interdisciplinary meetings, facilitate referrals for community resources, and review critical access admissions and monitor patients with regularly scheduled admission visits Social Worker Qualifications: Must have a Bachelor's degree in Social Work Must have a valid LSW license or the eligibility to obtain one Previous experience in Hospice is a bonus Surrounding Location and Community: Tight-knit community where neighbors know neighbors Experience a lively and active town with specialty shops, local restaurants, a theater, and much more Join in multiple annual traditions hosted by the town throughout all points of the year
